Your free screen
What happens in the 20 minutes
You meet the Doctor of Physical Therapy who would actually treat you — not a tech, not a salesperson. The whole point is to answer one question: can we help?
Tell us what's going on
How it started, what you've already tried, what makes it better or worse, and what you want to get back to — the barbell, the trail, your kids, your job.
We take a look
A focused check of the movement, strength, joint and nerve findings that matter for your problem — enough to know what's driving it and whether it's something we treat.
You get a straight answer
Either "yes, here's what it would take" — with a plan and the cost up front — or "this isn't a PT problem" and who you should see instead. No pressure either way.
Spine to sport
What we screen for
Most clinics are good at one end of this spectrum. We built H.I.R.T. to handle the whole thing — from a herniated disc to a return-to-sport test.
Spine & nerve
- Low back pain and disc herniation
- Sciatica, nerve pain, numbness and tingling
- Spinal stenosis and degenerative changes
- Neck pain, headaches and radiating arm pain
- Pre- and post-op spine care
- "I've tried PT before and it didn't work"
Sport & performance
- Squat, deadlift, bench and Olympic-lift injuries
- Running, jumping and court-sport injuries
- Shoulder, hip, knee and ankle rehab
- Return to sport after surgery (ACL, labrum, rotator cuff)
- Recovery: dry needling, shockwave, PEMF
- Bridging the gap between rehab and training
Rehab inside a real gym. Our clinic sits inside Rise Gym — 37,000 square feet of racks, platforms, turf and sleds — so your rehab looks like the thing you're getting back to, not a hallway with a stationary bike.
What makes H.I.R.T. different
Not your traditional physical therapy
1-on-1 care every visit
No tech, no double booking. A full hour of focused, individualized treatment when you're a patient — and the doctor's full attention at your screen.
Spine & sport specialists
Ortho and sports board-certified clinicians who publish research and teach other physical therapists.
Performance-based rehab
We bridge the gap between rehab and training, so "cleared" means ready — not just pain-free.
Cash-pay model
No insurance restrictions on what we can do or how long we spend with you. Better care, faster results.

Questions people ask before booking
Is the screen really free? What's the catch?
It's free, and there's no obligation. The screen exists because most people aren't sure whether physical therapy will fix their problem — so we'd rather take 20 minutes to find out than have you guess. If we can help, we'll tell you exactly what it would take. If we can't, we'll point you to who can.
What's the difference between a screen and an evaluation?
A screen is a focused 20-minute look to decide whether you're a good fit for care here. An evaluation is the full 60-minute visit where we test everything, treat hands-on, and build your plan. The screen is free; the evaluation is our normal first visit.
Do I need a referral from a doctor?
No. Texas allows direct access to physical therapy, so you can start with us without a physician referral. If it turns out you need imaging or a specialist, we'll tell you plainly and help you get there.
Do you take insurance? What does it cost if I continue?
Nothing is required after the screen. If we think we can help and you want to move forward, we'll walk you through exactly what it would involve and what it costs before you decide anything. We're a self-pay clinic — that's what lets us spend a full hour with you one-on-one — and FSA/HSA cards are accepted.
What happens after I submit the form?
We'll call or text within one business day to find a 20-minute slot that works. Prefer to talk now? Call or text (281) 702-4262.
