Harris Institute for Rehabilitative Therapies ("H.I.R.T.," "we," "us") is a physical therapy practice located inside Rise Gym at 11703 Spring Cypress Rd, Tomball, TX 77377, and operates the website www.hirt-pt.com. This policy explains what information we collect when you visit our website, request a free session, screen, scan, or evaluation, book an appointment, contact us, or respond to one of our ads, and how we use and share it. It also covers the emails, texts, and calls we exchange with you.

Medical records are covered separately

This policy covers our website, online forms, and marketing. Once you become a patient, the health information we create or receive while treating you is protected health information under HIPAA and Texas law and is governed by our Notice of Privacy Practices, which we give you at your first visit and will provide on request. We do not use your treatment records for advertising without your written authorization.

Information we collect

Information you give us

  • Request and contact forms (for example, a free recovery session, free injury screen, free 3D body scan, discounted evaluation, or a general question): your name, email address, phone number, the treatment or service you are interested in, what is bothering you or what you want help with, and anything else you choose to tell us.
  • Lead forms inside Facebook, Instagram, or Google ads: the platform sends us the details you entered, typically your name, email, phone number, and your answers to the form's questions.
  • Online booking (through our scheduling platform, PtEverywhere): your name, contact details, date of birth, appointment details, intake and consent forms, insurance or payment details if you provide them, and your appointment history. Payment card details are collected and processed by PtEverywhere and its payment processor; we do not store full card numbers on our website.
  • Mentorship and other inquiries: your name, contact details, background, and your message.
  • Other communications: anything you include when you email, text, call, or message us on social media, or when you leave a review.

Information collected automatically

When you visit our website, we and our service providers collect information about your device and how you use the site: IP address, browser and device type, pages viewed, links clicked, the page or ad that referred you, approximate location, and the date and time of your visit. We collect this with cookies, pixels, and similar technologies, including:

  • Squarespace (our website host): site functionality and basic visitor analytics.
  • Google Ads tag: measures whether our ads lead to visits, calls, texts, and form submissions, and may be used to show you H.I.R.T. ads on Google and its partner sites (remarketing).
  • Meta Pixel: measures the results of our Facebook and Instagram ads and may be used to show you H.I.R.T. ads on Meta platforms.
  • Embedded content from Google Maps, Instagram, and our booking platform, which may set their own cookies.

When you arrive from an ad, our forms also record which ad, campaign, or search term brought you here so we can tell which ads work.

How we use information

  • Respond to your request, schedule your session or appointment, and follow up with you by text, call, or email.
  • Prepare for your visit, for example by noting the treatment you picked or the area you want looked at.
  • Create and manage your appointments, intake forms, billing, and account.
  • Send service messages about your appointment, our hours, or our location.
  • Send marketing about our services, programs, and offers by email or text, where permitted and until you opt out.
  • Understand how people use our website and ads so we can improve them.
  • Protect the safety and security of our patients, staff, facility, and systems; prevent fraud; and enforce our agreements.
  • Comply with legal obligations and respond to lawful requests.

Text messages

If you give us your mobile number, we may call or text you about your request or appointment. Texts may come from our clinic line or from a team member's mobile phone. If you opt in to marketing texts, message frequency varies, and message and data rates may apply. Reply STOP at any time to stop receiving texts, or HELP for help. You can also email sean@hirtpt.com. We do not share your mobile number or your text opt-in consent with third parties for their own marketing.

How we share information

We do not sell your personal information for money. We share it only:

  • With service providers that work on our behalf: website hosting and forms (Squarespace), scheduling, intake, and billing (PtEverywhere), payment processors, email and text messaging providers, and cloud storage and productivity tools (Google, including Google Sheets and Gmail, where form submissions are stored and routed to our team). They may use your information only to provide services to us.
  • With advertising and analytics partners (Google and Meta) through the cookies and pixels described above. We do not send them your treatment records. Under some state laws this counts as "sharing" for targeted advertising; you can opt out as described below.
  • When required by law, such as in response to a subpoena or court order, or to protect the rights, safety, or property of our patients, staff, or others.
  • In a business transaction, such as a sale, merger, or reorganization of our practice.
  • At your direction or with your consent.

Rise Gym, LLC operates the gym we are located in as a separate business with its own privacy practices. Information you give Rise Gym is not covered by this policy, and we do not share your information with Rise Gym unless you ask us to.

Cookies and your choices

  • Browser settings: you can block or delete cookies in your browser settings. Some site features may not work without them.
  • Google: manage personalized ads at adssettings.google.com. Learn how Google uses data from sites that use its services at policies.google.com/technologies/partner-sites.
  • Meta: manage how Meta uses information from businesses in your Facebook or Instagram ad preferences at facebook.com/adpreferences.
  • Industry opt-outs: optout.aboutads.info and optout.networkadvertising.org.
  • Do Not Track / Global Privacy Control: our website does not currently respond to these browser signals. Use the opt-outs above instead.
  • Email and texts: use the unsubscribe link in any marketing email, or reply STOP to any text.

Your privacy rights

Depending on where you live, you may have the right to ask us to tell you what personal information we have about you, correct it, delete it, give you a copy of it, or stop using it for targeted advertising. To make a request, email sean@hirtpt.com with "Privacy request" in the subject line, or call (281) 702-4262. We will verify your identity by matching the details you provide against our records and respond within the time required by law (generally within 45 days). If we deny your request, you may appeal by replying to our response. We will not treat you differently for exercising these rights. Some information may be exempt, for example medical records we are required to keep under Texas law, or records we must keep for billing, legal, or safety reasons. Requests about your medical records are handled under our Notice of Privacy Practices.

Children

Our website is intended for adults and is not directed at children under 13, and we do not knowingly collect personal information online from children under 13. We treat minors only with a parent or guardian's consent, and we collect their information from the parent or guardian. If you believe a child under 13 has given us information through our website, email us and we will delete it.

How long we keep information

We keep personal information for as long as we need it for the purposes above: inquiries and session requests for as long as it is useful to follow up with you, and patient records for the period required by Texas law and our licensing board, plus the period required for billing, legal, and dispute purposes. After that we delete or anonymize it.

Security

We use reasonable administrative, technical, and physical safeguards to protect your information. Our website is hosted by Squarespace and served over HTTPS, form submissions are stored in access-controlled Google accounts, and scheduling and payment data are handled by PtEverywhere and its payment processor. No method of transmission or storage is completely secure, so we cannot guarantee absolute security.

Links to other sites

Our website links to third-party sites and services, including our PtEverywhere booking portal, Facebook, Instagram, and Google Maps. Their privacy policies govern your use of those services.

Changes to this policy

We may update this policy from time to time. We will post the new version on this page with a new effective date, and for significant changes we may also notify you by email or with a notice on our website.
